Gary Jacobs


Gary is a seasoned health care executive with a breadth of experience in the health care insurance and health care products market including Medicare Advantage, Medicaid, Medicare Supplement, Post-acute care, Long Term Care, exchanges, and payer-provider collaborations.  He has a diverse history of identifying new business opportunities, developing long term growth strategies and strategic alliances, and implementing enterprise-wide initiatives for health plans and post-acute and long term care providers.  Most recently, Gary served as Executive Vice President for strategic relationships for CareCentrix, where he was responsible for accelerating CareCentrix’s transition to value-based care through external partnership development.  Prior to joining CareCentrix, Gary was a Managing Director in the Heath Industries Advisory Practice at PricewaterhouseCoopers (PwC) where he led the National Government Programs practice.

Gary's experience includes starting up a successful management company specializing in long term care systems, innovating substance abuse and mental health outpatient programs in HMO settings and providing technical assistance to HMOs for these programs.  He has also lead enterprise-wide planning initiatives, strategic alliances, and government affairs for a Medicare Advantage plan where he was instrumental in identifying acquisitions and new business opportunities including: the acquisition of a Part D company and a population health management company serving Medicaid populations with particular emphasis on behavioral health and managed long-term care.
